What we collect

When you use the consultation request form on this site, you share your name, phone number, the treatment you are asking about, and any details you choose to add such as insurance information, urgency, or notes about your situation. We do not collect information you do not submit.

How your request is sent

The form prepares your request as an email addressed to our appointments inbox. Your email app opens with the request pre-filled, and nothing is transmitted until you press send yourself. If your device has no email app configured, use the copy option in the confirmation message instead.

How we use it

We use your information only to respond to your consultation request: to contact you by phone, text, or email, to understand what treatment you are asking about, and to schedule a consultation if you choose one. We do not sell your personal information, and we do not share it with advertisers or data brokers.

Health information

Please share only what is needed for us to understand your request, such as symptoms or timing. Do not submit full medical records, imaging files, or Social Security numbers through the form. Detailed health history is collected securely at the practice if you become a patient.

Patients under 18

If the patient is under 18, a parent or legal guardian should submit the request and will be the contact for scheduling and consent.

Your choices

You may ask us to correct or delete the information you submitted by replying to any email from us or by submitting a new consultation request with the words “privacy request” in the notes field. We will act on verified requests promptly.

Changes to this policy

If this policy changes, the updated version will be posted on this page with a new last-updated date. Continued use of the form after changes means you accept the updated policy.
